In an effort to further ensure the safety of our attendees, all 2021 Annual Conference attendees are required to be either vaccinated against COVID-19 OR provide verification of a negative COVID-19 test result within the 48 hours immediately preceding arrival at the conference. Any false statements may be forwarded to our Committee on Professional Conduct for possible ethics violations.

All new registrants will provide their selection during registration. All existing registrants must log back into their record and access their registration and provide their response. Please see the images below for further guidance.
